Questions
Which lists do you check?
the OIG list and SAM.gov, plus 15 state Medicaid lists: Pennsylvania, New York, New Jersey, California, Georgia, Missouri, Indiana, North Dakota, Iowa, Arkansas, North Carolina, South Carolina, Maryland, Kentucky, Montana. The state map above shows each state. Each record names every list it checked, the date of each copy, and any list it could not check.
How often?
Your first check runs as soon as you add your people and vendors. After that it runs on its own every month, against the latest copy we hold of each list. We fetch each list about once a day. You can also run one extra check a day.
What happens on a possible match?
When an NPI or date of birth agrees, it is marked as a match to act on. When only the name agrees, it is marked for you to confirm, with the reasons and how to check it with the list publisher's own search. You record your decision, with your name and how you checked, and it appears in the next record.
Can I show the record to an auditor?
Each check is kept as a dated, printable record: which lists were checked, the date of each list, every name and its result, and any decisions you recorded. You can download every record in one file, with a check that none was altered.
Is this legal advice?
No. For your situation, ask your attorney or compliance adviser.
What do you keep about my staff?
The names you add, and any role, NPI and state you give. A date of birth is never stored as written, only a coded fingerprint used to compare it. The privacy page lists everything we store.
How it works, with the numbers
The lists. On 25 Oct 2026 the service holds 17 lists: the OIG list (84,093 entries, copy dated 2026-10-09), SAM.gov (168,992 entries, copy dated 2026-10-23) and 15 state Medicaid lists. A list copy too old to rely on is named in the record as not checked. SAM.gov rarely lists an NPI, so its close names rest on name plus city and state, and each one needs a person to check it.

The board above. It runs ClearRoster's own screening code in your browser, against made-up lists. Compare it with the service's live sample record.
The record. Each month's record is dated and carries a SHA-256 fingerprint. Change one character and the fingerprint changes, so an auditor can see nothing was edited. The recording above was made on a test copy of the service, so its names, lists and fingerprint are test data.
The penalty ceiling. Up to $25,595 per item or service (2025 maximum, 42 CFR 1003.210(a)(4), Federal Register 28 Jan 2026), plus up to three times the amount claimed. OIG sets the real amount case by case; a Florida podiatry practice settled for $41,739.66 in January 2025 over an excluded receptionist.
